In my 12 months as a Topol Digital Fellow I will establish a digital pathway combining patient initiated follow up (PIFU), remote monitoring and educational resources to enable our OP service to empower self-management and see the right patients at the right time.

My Topol fellowship problem / project:

People with chronic skin conditions are often the most knowledgeable on their condition. I want to expand on this through implementation of a digital pathway to empower patients to monitor and take care of themselves where feasible and have clinician input when convenient and relevant.

I am an ST5 in dermatology that has been served very well as a patient within dermatology services. I endeavour to improve current services and ensure our dermatology care remains digitally up-to-date.
